New Patient Registration

  1. Complete our New Patient Registration Form.

  2. Click on the “Patient Portal” button (top right corner of this page)>“Create an Account”> email verification

  3. Accept all: in notification settings (required)

  4. Click on “contact information”>edit>complete all fields>submit

  5. Upload a picture of yourself and your insurance card to the portal.

We do not prescribe controlled substances, including pain medications, sleep aids, and stimulants to treat ADHD.
